The increasing use of AI-powered chatbots for mental health support is rapidly reshaping how individuals seek guidance during difficult times. With platforms like ChatGPT offering tailored “therapist” bots, many individuals are turning to these tools for solace, advice, and even coping strategies. But while AI chatbots are becoming an accessible option, mental health experts are urging users to consider the potential risks and limitations.

When ChatGPT introduced the ability to create custom “GPTs” in 2023, it opened a new frontier in digital therapy. These bots offer a safe, non-judgmental space for users to share their thoughts and receive coping strategies. One user, Mya Dunham, shared her experience with the chatbot, explaining how she uses it for emotional support twice a week.

“I wasn’t expecting it to feel so human,” said Dunham, who first used the AI after seeing someone share a positive experience on social media. She now finds herself turning to the chatbot for perspective on her feelings, finding the responses comforting and without judgment.

However, the advent of AI therapy tools is a double-edged sword. While the chatbots provide convenience and an accessible outlet for support, mental health professionals stress that these bots are not replacements for human therapists.

According to Dr. Russell Fulmer, Chair of the American Counseling Association’s Task Force on AI, AI chatbots can be helpful in managing mild mental health conditions like anxiety and depression, particularly for populations who may feel more comfortable opening up to a machine than to a person. However, Dr. Fulmer emphasizes that chatbots should not be used as standalone solutions.

“While AI chatbots can offer comforting advice and act as an outlet for emotional expression, they do not have the ability to address the underlying complexities of mental health issues,” said Dr. Fulmer. “The best use of chatbots is in conjunction with traditional therapy, where a licensed professional can provide context, address misconceptions, and help guide the user’s mental health journey.”

Despite their limitations, AI chatbots are proving to be invaluable tools in certain contexts. They are particularly useful for individuals who may not have access to therapy due to cost, insurance issues, or scheduling challenges. Free-to-use chatbots, available 24/7, provide immediate support and accessibility that traditional therapy may not always offer.

“Chatbots can serve as a valuable first step for those who are struggling and need someone to talk to, even if it’s a machine,” said Dr. Daniel Kimmel, a psychiatrist at Columbia University. Dr. Kimmel conducted a study comparing ChatGPT’s therapeutic responses to those of a human therapist and found that while the chatbot excels at validating a user’s feelings and offering general advice, it lacks the deeper insight and inquisitive nature a human therapist would provide.

“Human therapists are not just listeners—they are also interpreters, connecting the dots between a patient’s history and present emotions,” Dr. Kimmel explained. “AI, while helpful, lacks this nuanced understanding.”

Despite their growing popularity, there are several risks associated with using AI chatbots for therapy. Dr. Marlynn Wei, a psychiatrist based in New York City, cautions that general AI chatbots may not be equipped with the necessary safety protocols to recognize when a user’s situation requires professional intervention.

“Chatbots are not designed to handle crises or identify when a user needs to speak to a clinician,” Dr. Wei said. “There’s also the risk that chatbots could provide inaccurate or biased advice, especially when it comes to sensitive mental health issues. AI still struggles with ‘hallucinations,’ meaning it can generate responses that are misleading or incorrect.”

Moreover, AI chatbots lack the empathy and emotional depth that a human therapist provides. While chatbots can offer advice and a sounding board, they cannot replicate the emotional intelligence that a trained therapist brings to a session.

There are additional concerns for vulnerable populations, such as children and minors. Chatbot platforms like Character.AI have faced lawsuits due to inappropriate content being directed at young users, with some claiming that chatbots encouraged self-harm or violence. These incidents underscore the importance of safeguarding users, particularly those in sensitive or high-risk situations.

Dr. Fulmer recommends that minors and those in crisis should not rely on chatbots as their sole source of support. “While chatbots can be a valuable tool, they should be used with guidance, especially for vulnerable individuals,” he said.

While AI chatbots are still in their infancy in the mental health sector, their potential as supplementary tools cannot be ignored. They offer a scalable solution for accessible, affordable support, especially in situations where professional therapy is not immediately available. However, mental health experts are clear that these tools should complement, not replace, traditional mental health care.

“AI chatbots can offer important support, but they cannot replace the role of human therapists in addressing the complexities of mental health,” Dr. Wei concluded. “The future of mental health care may include a combination of both AI tools and human professionals working together to provide the best outcomes.”
