In an effort to bolster mental health and wellbeing among residents of Residential Aged Care Facilities (RACF), mental health service providers are invited to collaborate with the HNECC PHN (Hunter New England Central Coast Primary Health Network). A Request for Proposal, slated for release in early August, seeks qualified providers to implement evidence-based interventions using a stepped care approach. The focus will be on addressing mild to moderate mental health conditions, particularly those arising from the transition into aged care.
The initiative emphasizes the adoption of integrated and innovative models of care, including digitally assisted interventions alongside traditional one-on-one approaches. This strategic shift follows a comprehensive evaluation by the PHN of existing mental health services in RACFs, highlighting the imperative to enhance accessibility to support services.
Key objectives of the Mental Health Services in RACFs initiative include:
Enhancing the efficiency and effectiveness of mental health and suicide prevention services for residents experiencing or at risk of mental health disorders or suicide.
Implementing integrated, system-based suicide prevention activities in collaboration with Local Health Districts (LHDs) and local organizations, supported by the development of a joint regional mental health and suicide prevention plan, service mapping, and needs analyses.
Central to achieving these goals is the engagement of the Residential Aged Mental Health Program, fostering ongoing relationships with local General Practices, Allied Health Professionals, RACF services, and other stakeholders. The initiative aims to establish clear health pathways, ensuring that referrals can be made effectively by GPs, Allied Health Professionals, Nurse Practitioners, RACF staff, and family members.
This collaborative effort seeks to significantly reduce mental health disorders and suicide rates among RACF residents, promoting a supportive and resilient community environment.
